Treatment using implants is not uncommon these days. However, it is still not common.

Implant treatment tests the comprehensive capabilities of the dental clinic. Simply having an implant is not enough. Specialist knowledge is required over a wide range of areas (implant roots, teeth, occlusion, maintenance, etc.).

イo prevent pain and swelling during implant surgery, you will be asked to take medication the day before. During surgery, drills that are normally used to adjust bones are rarely used. The bone is adjusted using a special instrument that matches the shape of the root of the implant. This is very effective when the bones are soft and can improve the properties of the bones. Improving the bone allows the root of the implant to be firmly fixed.

For the root of the implant, we use HA-coated implants, which have excellent adhesion to the bone. It is characterized by shortened treatment period and good stability. For patients who previously thought it was impossible to place the root of an implant without bone grafting due to a small amount of bone, it may be possible to do so without bone grafting.

At our clinic, we do not rely on simulation software to accurately measure CT images during diagnosis. On the day of surgery, we will interpret the image to the point where we can clearly see all the details, including the location of bones and nerves. The current situation is that more time is spent on pre-operative diagnosis and image interpretation than on the actual surgery. If the total is 100%, the pre-surgical diagnosis is 60%, and the actual surgery is 40%.

At our clinic, we do not compete for the number of cases, especially when it comes to implant treatment. Rather than increasing the number of cases, we prioritize ensuring that each patient is treated individually and that the patient is happy with the treatment. We basically introduce implant treatment as one of the treatment methods, and we will never forcefully recommend it to patients who are resistant to implant treatment.

What is HA implant?

A globally proven implant surface coating.Dental implants were developed in Northern Europe (Sweden) and have greatly developed in the West (Europe and America). The mainstream is titanium implants. Implants are foreign bodies to living bodies, so one important point is how to minimize foreign body reactions. The greater the foreign body reaction, the longer the healing period tends to be.
Asians and Japanese people with thin bones may not be compatible with titanium implants. This is especially noticeable in women who have low bone mass or who want to place an implant immediately after tooth extraction.
The HA implants used at our clinic are made of titanium coated with HA (hydroxyapatite), which has the ability to guide bone. Due to the osteoinductive ability of HA, the root of the implant actively integrates firmly with the bone immediately after surgery. Is there any pain during implant surgery?

There was no pain at allImplants are very large-scale work, and when I heard that the screws (the implant body) are inserted into the bone, I had the impression that it would be very painful, but they gave me proper anesthesia so I didn’t feel any pain at all.
However, there was some vibration when inserting the screws. Those who have had their teeth ground due to cavities may know that the vibration is not much different from the vibrations that occur when grinding. In addition, the operating room was thoroughly disinfected, so I was able to undergo surgery with peace of mind.

Is there any pain after implant surgery?

I didn’t feel any painAfter the surgery, I took painkillers before the anesthesia wore off, so I didn’t feel any pain at all. For 2 or 3 days, I felt more discomfort than pain. For about a week, I tried to avoid touching my teeth by eating with my opposite tooth and eating soft foods, so I was pain-free.

How long should I go?

I went to the clinic 4 times for 1 month follow-up, and I enjoyed going there.I had two implants installed to replace my lower molars. I had received an explanation of the general process in advance, but I had an impression that it would be a big project, so I thought it would be a long one, but I actually went through it four times. After the first surgery, there was a period of about a month to see how the gums had healed.
Once the gums have recovered, the next step is to create a foundation and a covering. I think that the treatment for cavities is similar to that of grinding down the teeth or taking impressions of the teeth. I was nervous during the first surgery to insert the screws, but after that, I was happy to see the teeth gradually form, and I looked forward to each visit.

Have you actually tried using an implant?

Five years have passed since treatment, and the bite remains comfortable and clean.They are no different from normal teeth, so you can eat a lot of hard foods! I had no teeth for a long time, and the tooth next to me was falling down. If I left it like this for a long time, my other teeth would become damaged and my bite would become more and more irregular. I thought I was missing two of my back teeth, so I ignored them, but when I learned that just missing those two teeth would have a negative impact on my other teeth, I realized once again how important teeth are.
At first, I considered a treatment called a bridge, in which the left and right teeth were shaved down, but I was very reluctant to shave off healthy teeth. Also, I had the option of using dentures, but I heard that my chewing force was almost 70% less than that with healthy teeth, so I quickly decided against using dentures.
It has now been 5 years since I had the implant. It feels good to chew, and since it looks like white ceramic, it has been able to maintain its clean condition, and I feel that it was a good thing for the future. Why is it necessary to decide on a treatment plan before tooth extraction?

In the case of front teeth, if a tooth that has been diagnosed as unsalvageable is suddenly removed, bone resorption begins from that moment.

If the bone resorbs excessively, it will be difficult to repair it beautifully.

At our clinic, we prevent bone resorption by placing an implant at the same time as tooth extraction or by filling the tooth with a bone filler that regenerates the bone. In most cases, the bone that firmly supports the teeth regenerates within about 6 months.

After confirming bone regeneration, if the treatment plan is to use an implant, we will perform implant treatment.

In the case of molar teeth, at our clinic, we extract the tooth that cannot be saved, and at the same time, we explain the treatment plan in advance, fill it with a bone-filling material to regenerate the bone, and then close the tooth with sutures.
In most cases, the bone that firmly supports the teeth regenerates within about 6 months.

After confirming bone regeneration, if the treatment plan is to use an implant, we will perform implant treatment.

I need a tooth extracted, but before that…

If you remove a tooth that cannot be saved and leave it as is, the bone that supports the tooth will be severely resorbed.

As a result, if new bone formation is required for implant treatment or if dentures are used, the dentures become unstable.

つTherefore, when considering post-extraction treatment and restoration, it is important to minimize bone resorption.
Retaining blood clots is important to speed up healing and prevent bone resorption after tooth extraction. This blood clot becomes a stepping stone for healing.

At our clinic, we recommend using a bone replacement agent (BIO-OSS, which will be replaced by bone in about 3 to 6 months) when tooth extraction is performed.

If only the tooth is extracted, bone resorption will be greater. We recommend the use of bone grafting agents to maintain future health.

If a tooth needs to be extracted, it is very important to decide on a treatment plan before the tooth is extracted.

You need to decide whether to treat with an implant, restore with an interlocking crown, or recover with a denture.

At our clinic, we will discuss the treatment plan during the pre-extraction consultation, and once the treatment plan has been decided upon, we will perform the tooth extraction.

Regular X-rays are taken from one direction and only reproduce simple shadows. For implant treatment, it is extremely important to have an accurate three-dimensional understanding of the treatment area.
ndispensable for this accurate understanding is state-of-the-art dental CT. CT performance has improved dramatically in recent years. Capable of imaging and analysis in 1mm increments, the area where the implant is planned to be placed is reproduced very clearly, making precise analysis possible.

The root part of the implant is often used with a diameter of approximately 4 mm and a length of around 10 mm, so a difference of 1 mm has a very large effect. State-of-the-art CT produces very clear images, allowing for extremely precise interpretation and diagnosis.

However, this state-of-the-art CT is very expensive, and there are still only a few installed nationwide.Also, as it is an ever-evolving and cutting-edge field, updates are very quick. For the above reasons, our clinic performs CT scans in a location with state-of-the-art equipment.
